What is Final Forms?

The Ravenna School District has engaged with Final Forms as its forms management system for registration for students and athletes.

Students and parents are required to use Final Forms to sign and complete all forms necessary for athletic participation.  All students must complete the Contact Information, Health History & Medical Profile (Which includes Medications, Injuries and Hospitalization and Past & Ongoing Health Conditions), Emergency Medical Authorization, Acceptable Use Agreement, Permission to Photograph/Videotape and Universal Field Trip Permission.

Athletes will be required to complete the following forms in addition to the ones mandated for all students:  OHSAA PPE Physician Questions, Athletic Code of Conduct, Acknowledgement of Risk, Sports Passes & Tickets, Wash and Care Instructions, Senior Night Information, OHSAA Preseason Meeting Presentation, OHSAA Student Eligibility, Sudden Cardiac Arrest Awareness (Lindsay’s Law), Concussion Acknowledgement, ImPACT Testing Consent, OHSAA Authorization and the OHSAA PPE Physical (for Physicians).

All of the forms must be completed before the first official day of practice for each sport.  Coaches now have the ability to view who is eligible based on who has their forms completed when that day comes.  If an athlete does not have their forms completed, the athlete will be requested to do before they will be permitted to practice.  If there is not a physical on file, or the physical has expired before the first day of practice, the student will be ineligible to participate until the physical has been received by our Athletic Office.  If the physical expires during the season at any time, the student will then be declared ineligible until the physical has been received by our office.
